I think if you want to bind two buttons you have to press them both at the same time, not sure.
An easier way to do it if you want to use smart cast regularly is just to bind them over your QWER keys and use it as standard.

You'd have to hold them both at the same time, but you should be able to start pressing them at different times. I doubt it will work, though. Generally, games only allow you to bind multiple keys when all but one of those keys are combinations of Ctrl, Shift, and Alt. So you could bind Shift + Q, Ctrl + Q, or Shift + Ctrl + Q, but not Tab + Q. As Luther suggests, a better solution is just to rebind smartcasting on the regular QWER keys. You can also rebind the normal click-to-target casts to Shift + Q through Shift + R, if you want to keep them around.
