My first two items are always the same, but depending on the enemy team I choose many paths based on the enemy players, their damage output and "CC", let me explain.
BOOTS
Very simple, you are a tank, your passive shield will max very easily so getting magic pen boots does very little, stick to merc treads vs heavy cc/ap teams, ninja tabi vs heavy melee, or swiftness if you feel they are more your play-style.
VS MELEE ONLY
Very obvious, first item should be thorn mail unless you can afford to buy warmog's armor (only if they don't have a kog-maw) once you have those two, upgrade your boots, then randuin's omen, followed by guardian angle, and last item is your choice, frozen mallet if they seem to be running a lot, or a sunfire if you are facing heavy teamfights.
VS CASTERS ONLY
First item before even boots is always force of nature, followed by merc treads then banshee veil, then work on warmogs if you like or leviathan if you are doing well, followed by a guardian angel, and I like to get rylai's scepter which allows your abilities to slow the enemy (works better than frozen mallet vs casters since your spells have a longer range than melee. Lastly buy a randuin's omen just for it's slow so that your team is able to clean up team fights.
MIXED
It would be very hard to tell you exactly what to buy, but just know this, stacking pure hp (sunfire mord) rarely works because you are neglecting his passive. Let me put it this way, his passive is more effective if you take less damage because the shield will be used up not quite as fast or even at all, so focus on countering their team by buying armor and or magic resist items, and their team should have a very hard time killing you

Keep in mind that as you get more and more magic resist or armor the % reduced damage taken from magic damage or physical goes down per magic resist or armor. To put it another way, say you get 100 MR/Armor and it gives you 20% reduced damage taken, then you get another 100 so now you have 200 MR/Armor but you only get 30% reduced damage taken. With that in mind try not to pass 75% reduced damage taken with your shield up or it becomes redundant and a waste of money. after you reach 75% focus on getting more damage or HP.
