unnecesary sword of the divine is unnecesary.
boots 3 are just bad.
phantom dancer is excellent on kog with malady/madreds, don't be skeptical.
banshee's is an awesome build on about 90& of champs, kog included.
I prefer 9/0/21 with attack speed instead of cooldowns.
consider a late game bloodthirster to be a hybrid ad/ap and keep alive with lifesteal - should be at about 2.3 attacks per second so the AD increase is very efficient.

I understand that you want maximum survivability for Kog'Maw, but warmog's really isn't that great of a choice. Kog'maw stays mana hungry all game, and if he gets CC'd late game you're pretty much guaranteed to die. I know you said that you don't like having to build for it, but Banshee's is one hell of an item on Koggy. Solid build though! :D
