At start you MUST feed your Siphoning Strike. After you stack it up to about ~100+ extra damage you can begin killing. Now after you at least get four items mention (everything without Phantom Dancer and Infinity Edge) and stack up Sword of the Occult your Siphoning Strike should hit about 600. Yes, it is quite real.

The only seeming flaw is the begging. If you are up against a tough meele opponent you might run out of mana. So if you have such problems I would recommend replacing Ignite with Clarity. Or if you are patient then you won't have such problems.

Now you are probably asking "Why Exhaust AND Wither?". The answer is simple, just imagine the opponent slowing 90%. Even Ghost wouldn't help.

"Why do you abandon Spirit Fire?". It may only seem useful at the start, but it has majorly messed up things. First, it depends on Ability Power, and yet it only benefits from about 25% of it, so it will never become powerful. Second I don't think you would find anyone stupid enough to stand and burn in the fire.

This is not a tank built, but I believe this kind of Nasus would down a tank Nasus.
