Info on how to play as twitch is below. (Gameplay, Items, Masteries, Runes, and just some good description! :)
Ok, I want this to be a simple build and guide but it won't be short. Twitch doesn't need attack speed items for awhile because of invis and ultimate attack speed bonuses. Stay invis and behind the enemy during a gank and you can easily put them to death. If they are physical then exhaust and if you are having trouble with the fight you picked the wrong battle. Although you picked the wrong battle you can still use flash as you try to invis. Make sure to flash to the nearest bush or ally area away from the enemy champion. For team fights you always want to make sure none of the enemies have an oracle. This will allow you to get by them and get into a perfect line the using of your ultimate. This will usually guarantee a win to the team fight. I am telling all of you from personal experience that this build WILL work. Maybe not all the time but it has been very successful for me in the past. My best score is 27 kills and 1 death in a 5v5 premade. Twitch can be pretty easy to counter considering he is a squishy as he is and they just need an unexpected ward or an unnoticed oracle.
Like I said, focus attack damage in the beginning. No malady although it can be good for expunge you have your "Q" ability (Leaving invisibility) and your ultimate. Start out with a doran's shield and a health potion. Get boots and a scepter when you can and then get the boots needed for the situation. Berserker Greaves are highly recommended for the attack speed. If there are a few stuns then you should get mercury treads. Get the B.F. Sword as soon as you can after. Work your way to a blood thirster and then an infinity edge as soon on after if you got the berserker greaves. If you got the mercury treads then you should get a recurve bow either before the blood thirster or after it. Get the last whisper which works well on all. If there are tanks you should get the madred's bloodrazor after, but if there isn't then get a phantom dancer. The last item should be a survivability item. I like banshee's veil but you could just get some more damage and health with the frozen mallet. If completely necessary then you could try warmog's armor for the massive health and health regen.
The build would be like this -
Doran's Shield - Good health and health regen
Berserker Greaves - Nice attack speed that is well needed early game
Mercury's Treads - Good to get out of stuns and some magic resist
Bloodthirster - Awesome lifesteal and damage for attacks
Infinity Edge - Also great damage but along with some crit
Last Whisper - Fantastic armor penatration along with the needed attack speed
Phantom Dancer - Good crit, move speed, attack speed, and also some dodge
Madred's Bloodrazor - Needed for tanks or enemys of high hp
Now, sell the doran's shield a survivability item! :D
Skill sequence - Honestly its your choice but thats kinda how I prefer it. I usually do mix it up every game though
Masteries - You can switch it up but once again, another preference I have
Runes - Not to sure but these work for the good attack speed and armor pen
