Rushing a bloodthirster is a risky move, since you got no resistances or health to help you survive It's true it is a risky move, however i've had the most success this way, if you feel unsafe doing so you can rush a phage and then go for the bloodthirster, having a fully stacked bloodthirster compensates for the lack of health with sustainability, and the resistances early on come from  Battle Roar.

Khatorn is right IMO. Rushing blood thirster doesn't seem like the most viable choice. especially say you go up against someone like nidalee or olaf. You are going to need health early for nidalee's poke and olaf's true damage. so Maybe getting more survivability before damage is a better choice. unless you play against people that dont know how to harrass very well.

Look guys i'm not forcing anyone to follow a order, i've said it before and i'll say it again you can go phage first then bloodthirster or frozen mallet then bloodthirster however your damage early will be lacking, rengar has almost infinite sustain with empowered w either way nidalle doesn't counter rengar due to the fact that although she can poke, rengar can come up close with q w e and damage her hard, however vs olaf its better not to pick rengar because true damage really hurts.
