@audiogreen - as i said in the guide, exhaust has more than a slow function. It reduces incoming damage and lowers resistance. Madreds would be useful if you actually used overheat that often. I generally avoid overheating if possible, but it cant be helped in team fights some times, and without an item to increase your effectiveness with your auto attack, you are basically useless. The reason I do not take Madreds over malady is because malady adds AP, AS, and it lowers MR, which is great in combination with your other items. While you may not be able to dive towers and continuously engage enemies, you will add incredible dps and team benefits (especially if your team is AP heavy).

I think you could have went differently on the exhaust combo, yes it will help you but with your Rylai's built it makes exhaust completely useless as it provides for the slow. Another thing to consider is Maladys, if you in any point in the game have to rely heavily on your overheating then melee attacks your team members must have seriously fed, or they must be really farmed. My Rumble ensures that i have already owned them if not fatally wounded them by the time i overheat.

Have you ever tried building him half tank half AP, but still AP heavy? thats what i do.
