Begining Game

This build is set up for soloing the top lane. It is the main reason behind maxing out his E ability first (after the first and third point going into his Q and W abilities of course). With the setup on the health per level runes with a little extra magic resist, you shouldn't be dieing much at all unless there is a stun gank. as soon as you have the gold to buy philosopher stone you go back and grab it to start your Gold per 5 count. All you have to do is be an annoying bully to the enemy top solo. If he can poke very well you are to turret hug and let your gold item carry you until 6.

Middle Game / Laning Phase

Okay, you have now reached level six. You may now be less cautious on what you do in lane, so long as you keep yourself at a healthy 100 to 150 mana. If your jungler hasn't already, he should be on his way to gank top and ease some pressure off of you. Use your flash to get a nice nockup and then smartly use your nockback to hit him into a good spot for your gank to finish him off at. If you must go under their tower don't be afraid. you have an amazing Ultimate to clear CC on you and reduce the majority of the incoming damage. So tank the tower for your jungler and get out without dieing. If this is done successfully, you are now a feared cow and wont want to be came near.

End Game

By now you should have the majority of your items, and are able to easily take all the poke and still be at a nice health amount to engage when the opening shows reveals itself. Having a FoN (Force of Nature) don't be so tempted to go back to base, you have plenty of health with the item selection which synergize well with the passive on FoN to regenerate your health enough to get you through until your next full item is ready. The only real problem you might have is with your mana. Having a teammate carry a Soul Shroud will help out... Or if your team doesn't mind feeding you the blue buff from your jungle, or the steal from the enemy jungle.
Now with all this you should be able to give your carries the support needed to win the match!
