Absofruitly! Malady shines early game, because of its' relatively inexpensive build components, and also, because it synergizes so well with your Toxic Shot. The MR shred means that your Poison will tick for even more damage, the attack speed bonus means that you can better kite the opposing toplane, and the extra AP adds some noticable damage to your Toxic Shot. Plus, building Malady early is great, because it will be super effective against most other solotops, as you can easily build this, before many of the opposing solotops can get their Mercury's Treads. This will make them take loads of extra damage, and will make them take some pretty significant damage, even after they nab some magic resist.
+1 to the guide. Although the guide isn't finished (please tell me you are going to write more xD), it certainly deserves to be on the top 5.

Isn't the title of your guide technically misleading? You've made an AD/Tank build, which means people are more likely to die from your auto-attacks and poison rather than your mushrooms. Dying on mushrooms imply that ****ton-damage-mushroom-as-you-go-to-bush-to-recall-on-low-hp AP Teemo.
But I actually have no qualms against this guide! It's actually quite nice. I would swap a few items around, though. Malady has a greater impact early game and Wit's End might be a more adequate substitution. Which brings me to my next point. You mention your items in the guide, but Malady isn't mentioned. Did you mean to put Phantom Dancer in the cheatsheet?
