This build provides decent survivability and DPS.

Early game:
Start with Boots and few hp pots. Rabid bite should be used most of the time(last hit or use on a champion if possible). When you are lv 2 , you can start luring enemies and trap them with your pillar of filth, this will give you a great oppurtunity for early game kills.
Don't overextend too much and try to think with your partner.

Mid game:
Always use contaminate in team fights to provide good chasing abilities and AS. Careful with placing the pillar and don;t let your own team mates get stuck because of it. Mind that you are not the easiest kill so act a bit like an off-tank.

Late game:

Always use your ulti on enemy tanks and bite down the squishies. Make sure nobody gets away when being slaughtered.

Situational Items:
When I'm fighting a heavy Tank team. I usually go for:
Madred's Bloodrazor & Last whisper instead of Phantom dancer and Hexdrinker.
I always go for Phage first tho.

When facing a heavy Caster team. I go for
Force of Nature instead of Phantom Dancer

When facing a heavy Melee team:
I choose:
Thornmail and Ninja Tabi
