This is my personal build for Twisted fate. I like it, as it has a large amount of lifesteal for survivability, with 2 malady's about 1.9 attacks a second, and roughly 315 damage with build completely finished and without the bloodthirster stacks.
With my masteries, I go 21/6/3, putting some in defence can help with a little extra armor and magic resist that can never go astray, and is always useful to keep you alive.
I start off by buying a dorans blade and a HP Pot (I think I accidentally left that out of my item build, first build I've made, sorry). I usually try to stay in my lane til around level 6, when I go back to buy beserkers greaves and a dagger. I then proceed to just farm minions and try to get any kills possible to become more fed (the extra damage and lifesteal from the dorans blade, I notice, really makes a difference, I get many more kills early game with that little extra damage and survivability).
By the time I usually finish my second malady, I attack much quicker (round 1.6-1.9 depending on level) and have 30% lifesteal, which will help keep you alive. Mind you, you do not do much damage until you get your first black cleaver, but the passive from the malady's adds up with the bonus damage. Mind you, alot of games end around this point for me, but incase it becomes a tough battle, I usually follow the rest of these items.
When building Black cleaver, I usually buy the BF sword first, to add some extra damage to my attack speed. Then I go for the long sword and finally finish of my black cleaver. Now the black cleavers damage bonus and passive mean that you will be able to take down enemies much quicker then before. The black cleavers passive, and the malady's passive add up to alot more extra damage ontop of your base damage.
From there, I either choose my second black cleaver for damage, or bloodthirster for survivability, but whatever one you don't choose, just build after your first choice.
Also, with the summoner spells, sometimes I go exhaust and ignite, and sometimes I choose exhaust and teleport or flash. I know that TF has a built in teleport, but when that is on cooldown and something happens, the second teleport is always useful. Plus when your early level and you need to recall and get back quickly, teleport is always useful.
