This build promotes massive Cooldown Redux. (It's best used when Soloing a lane, 2v1 or 1v1 doesn't matter). Once you hit lvl 9, you should be able to harass like crazy because of 1.8 or so second cooldown on your Transfusion. You should also easily get first blood, or kills by using a simple combo (This is during 1v1 situations) at lvl 6. Ghost + Transfusion + Hemoplague + Ignite (+ Transfusion if he is not dead) remember not to use your pool during attack unless you are sure you will get the kill and get away, save it for when you run back and since your ghost is still active you'll be one hella fast pool.

For team fights, remember Hemoplague is like having an extra person whacking them when used right, Spam Transfusion and your Tides of blood, you can also gank easily due to your high hp (tower diving) and heavy dmg output (Transfusion Spam). If used well you should have a lvl advantage on both Mid (solo lane) and all other players in the shared lanes. Considering that Vlad doens't need Mana, he can lane forever, so harass them away and keep eating up the XP.

If the game goes to late game you will become less effective due to people stacking magic resist, or everyone having very high hp and moving as a group. When that happens, stick with your team for team fights, because hemoplague is one annoying *** skill.

1v1 Combo (lvl 6) - Ghost + Transfusion + Hemoplague + ignite (+ Transfusion) when person is at half HP for squishy or 1/3 Hp for stronger chars.

Some people only use their pools to run away, your pool can save lives, as well as end them, when used with rylai, most people will be moving so slow they wont leave ur pool up until you're no longer a pool. Use it to slow enemies down so you can use Tides of blood and Transfusion on them again when they try to run away, or use them to slow enemies for ganks, or simply to run away.

Proper use of pool is usually what differentiates good vlad for bad vlads, saving it for a crucial moment is the right way to play, do not use pool on minions, it is a waste. Pooling with right timing can help u dodge anything. Pool can also be used to bait, stick around with low hp and when a person is about to come pool away so ur teamates can eat them.
