Another item that might be noteworthy against AA's, thornmail, giving a chunky 100 armour, as well as the AA return damage.
I generally test stuff more in 3v3, due to the shorter games, although it can have a negative impact in being too short, so really its just starting builds, which I think I can come to a close on for my personal opinion, so I'll prolly give 5v5 a go to try out some full builds later.
It sucks people vote guides down because they also have a Vayne guide or just because its not the same as every other build/their build.
It's not so much that you PLAY defensively with this build as you focus on having good defensive stats for late game so you can continue to be offensive with your bolts.
I've honestly never played 3v3 and never intend to. I came to LoL after playing DotA so 5v5 is just sort of what I do haha, but I would imagine the games would be a lot shorter in comparison to 5v5. I like your alternative item choices though, aside from Manamune. I can't recall ever having experienced mana problems after the very early laning phases of the game, but if you do have noticeable mana shortages then by all means go for it. Hextech, like you said, would really be the last thing you get because of the wasted stats. The only reason I would probably even consider getting it at all is because of the active. If it was just the AD/AP and lifesteal/spell vamp then I wouldn't even bother me thinks.
It's really a shame that people downrate the build without reading it because of a lack of AD. I guess I should try to make a note of the percentage damage in the title..
