Bot: Swain with Leona/Blitz/Taric/Janna/Alistar, Perfect counter against the meta AD+Supp bot.
Mid: Sivir/Caitlyn/Tristana, long range AD with great push and decent utility. Kog would probably fit aswell.
Top: Depending on the jungler and the other team, Preferably a assassin like Irelia, Akali, Talon, Kennen(good top counter), someone who can get those last hits in teamfights.
Even a pusher/bruiser like Mordekaiser and Yorick would work to I guess, due to their ultimate.
Jungler: Maokai/Skarner/Lee/Malph/Rammus, any kind of beefy jungler with good ganks, CC and who can initiate teamfights in mid/late game.
But the core in my premade is the Bot lane. Swain together with a Heave CC support is the easiest way to shutdown the enemy teams AD-carry, i.e. Swain and Leona is one of strongest lane combination in the game.
