Introduction
Here's the build I've been playing lately, it works pretty cool.
I like to play with this Masteries, the builds I use don't use conventional 21/0/9, 0/21/9 or that.
In offense I take the ArPen, so I can get more ArPen (That's obvious lol). I go for the 15% MPen because Yorick has some magic damage too. I don't pick Atk Speed, I think it's a waste of points.
In Utility I pick more Mana (A little bit damage from Manamune). That's pretty much it for the Masteries.
As said before, you can make your own changes, I'm just giving you my build.

Well... Let's start saying again, that you can build whatever you like and you can pick different items depending on the other team composition.
I like to start with Sapphire Crystal, so I don't run out of mana so fast. (The pots are obvious).
Then, I rush Sheen ASAP, even better if you pick 350g Boots. (Btw, you can build Tear first). With Sheen you are going to do a lot of damage with your Q on early.
After that, I get my Tear of Goddess to start building some mana, I finish my Mercury Treads.
Proceed to finish your Manamune and you are going to blow the enemies even harder.
Now start building your Atma's Impaler, Start with the Chain vest, I think it's better than starting with the Cloak of Agility. After you finish building it, Start building your Warmog's... Start with the Giants Belt if possible.
So now you're done, the last item I would take is a Banshee's Veil. (It gives you Mana and Health, boosting your AD with your Manamune and Atma's) (And of course, giving you a shield that blocks an ability. Also, you'll get MR, which is going to make you tankier).
The very last item you should get (But this would be in an extreme ultra late game (70 minutes?? I guess)) Is the Trinity Force.
I don't like to get Trinity Force ASAP, I think it's too much money. Maybe in other Champions I would do it but not in Yorick.
Always remember to buy Wards, buy some more pots if you're getting harrassed (2 ~ 3 Pots are useful).

I don't remember it exactly, It can be different from game to game.
You have to start with 1 point on the "E". Then level up the "Q". (You have to lvl R on lvl 6, of course). (You can give a point on "W" if you grab a Tear, so you can spam more skills building more mana).
I think it's more important to max this way: Q > E > R > W (Max Q, Then E, Giving points to R in every level possible, Finish the W).
"E" will heal you up on Early, it will be helpful so you don't have to go to base every time.
"Q" Is your main damage skill.
"W" Of course, the slow.
"R" Try to use this on yourself when you're doing insane Damage, otherwise, Use it on someone that your team needs more than you.

As you probably saw on my masteries, I pick Exhaust/Flash.
Exhaust is useful to run away or fight against someone mitigating its damage.
Flash of course, saves you from anything, use it to jump over walls. (Surprise attacks would work, but I wouldn't recommend it, Yorick does not have a hard CC, so I think it's useless).
You can use Flash to chase someone running away with low life.
You can use Ghost or Ignite too, but that would require a change on masteries.
It depends on yourself.
