*This is my first time at a build/guide. Any constructive feedback is appreciated!*
You can switch the first Enrage for a Gaze if you have a good partner and are going for First Blood. If that is the case, skip the Regrowth Pendent and pick up a Dagger to start. It all turns out to be the same in the end as you can use the Dagger for Zeal. Another alternative to Regrowth Pendent is Vampiric Scepter, however I prefer the passive regen better as you can heal while out of combat or in transit.
Get your Enrage up as a priority except when your ult is available (lvl 6,11,16)
I won't go too much into detail, because the items are explained more in the gameplay section. The idea is since you start by hitting hard and slow, working on hitting hard and fast will help you get those killing blows in the early game. This is done through the Berserker's Greaves and Zeal. Next you need a way to snare your opponents so you can finish them. Phage or Lizard Buff(from neutral monsters) works. At this point you should be swinging super fast. Its now time to make those quick swings truck and crit. You can either go for Tri-Force first to combine your Phage and Zeal together (Sheen is awesome for ganking. Gaze then melee for double damage, if its a crit its practically over) or Infinity Blade first. It's between your personal preference and the flow of the game that will decide the priority. After this point you should be unstoppable. To aid in the final push, make a defensive item and/or a lifesteal. Take your pick then end the game!
Early Game "You can run and tell THAT, homeboy":
You can solo lanes and hold those pesky 2v1's at bay. Ranged champions can be a bit annoying at first while you are laning. Gaze and hit them once and they will run home crying after 2-3 times of this. If a melee champion is foolish enough to try and rush you 1 v 1, fall back a couple steps then Gaze followed by Exhaust, only certain champs will make it out of that combo alive (barely).
If you are unfortunate enough to get a ranged partner who loves to snipe killing blows (which is ridiculously easy to do to Sion), don't stick around and take the abuse, on your next teleport pick a different lane and try not to hold a grudge (as you will be able to save their butt later).
If you get low on health and have a lot of mana, use your Cannibalism to get back to full and continue farming. If you are low (<20%) on mana then hold on to your Cannibalism and stay out until the next $$$ breakpoint before teleporting. **Watch for enemy cherry-pickers** Somebody on the other team will probably notice your low hp and mana and come running for a gank so be VERY paranoid. As soon as they come into view (STAY AWAY FROM BUSHES) use your Caress, pop Cannabalism, then Gaze. If it was a single assassin, you will teach them a lesson that they can think about for the next minute as they wait to respawn. If they start running away, do NOT chase.
Mid-Game "We lookin' for you, we gon' fiiiiiiiind you!": (Around level 9)
Mid game effectively starts for you once you have movement speed, attack speed, and a on attack snare. Start looking for favorable match-ups on the board (squishies). Use the jungle to travel (keep an eye out for junglers. They make good free kills) to a flanking position and unload a world of hurt. If an enemy with low hp is running to a tower you can chase em past the tower to finish them off with Gaze (you can take a good number of hits from a tower and can pop Caress). Just keep floating around and putting the fear of Sion into them. If a lane is abandoned, and nearby, pick it up and push it for fast gold/xp/hp. Don't get locked into going back and forth with an enemy champion, keep mobile and keep them in the dark. Once you get your Infinity Edge, the game has now officially transitioned into end-game.
TIP: A good gank strat is to hide in the bushes behind enemy lines as they push a lane, prep your Caress then burst out and smack em, then Gaze and kill. You can pop your Caress to clear minions (and get extra fire power) and to add a nice chunk of burst to your combo, or keep it up to absorb any retaliation (if there are 2 champs).
End-Game "He's climbin' in your window; snatching your people up!":
Depending on how many kills you get and how much time you get to farm a lane, you should reach this point around 11-13. Once you have your Infinity Edge, find a 1 v 1 lane, TP into it, murder the enemy champ and push the lane through the inhibitor. They won't be able to stop you unless they have 2-3 powerful champs (or 1-2 amazing ones). At this point flip to a different lane and solo down towers. Keep them constantly re-positioning. Once you have 2 inhibiter's down (in 5v5) then you can press into their base with your teammates.
