Items

At the beginning we need to earn a lot of money, so I usually buy 1 Philosopher's Stone and 2 Heart of Gold (before boots).
Then I buy the Mercury's Treads.
Ok, now we need an heavy armor and the Thornmail is the best choice, after that I'll buy a Sunfire's Cape.
So.. now I have no slot for other objects; no problem! I've to sell an Heart of Gold to buy the Force of Nature (that help a lot with his regen). Then I'll sell the other Heart buying the Guardian Angel.
Usually the game finish with this set (winning), but.. if the battle goes on, I've only to sell also the Philosopher to buy the Aegis of the Legion.

Tattics

Until you buy 1 Philosopher and 2 Hearts, you have to play a defensive match. Saving allies with Taunt.
Then you can play better (already tanking towers).
But the real Rammus begins after the Thornmail. With the Thormail, you can do the combo (taunt + earthquake + defensive ball curl). Use Powerball only to escape or if an enemy survive at your combo.
With the Sunfire you've to play alike (but works much better)
Buying the Force of Nature you have not to return in base so often.

Important: Try to use your Teleport on a minion under an enemy turret when enemies are far, with Defensive ball (that increase your damage) + Earthquake you will probably destroy the turret; then use Powerball to run away quickly
