Can't say much about this one, been using it for the last week and so far it works good. Get Cooldown Reduction in beginning so you can use Transfusion faster. Keep Sanguine Pool for hard times (when you're about to die). Sanguine Pool can also be used in team fights after you casted Hemoplague.

When your items are full, get rid of Doran's Shield to buy Will of the Ancients. If full again get rid of Visage to get a Rabadon's Deathcap. By then you should have around 800ap... and 5500hp.
