The doran's shield start would only be beneficial if you are a very aggressive support and plan on engaging into a fight pre-level 6. The health it gives you + its ability to block auto attack damage will make you slightly tankier than a relic shield would. But the down side is you wont get the slight healing and passive gold. But if your plan is to all in to the death early, you wont be needing the healing anyways so spending money on it would actually be a slight waste of gold. The relic shield is much better in passive lanes (sustain lanes). Since I typically fight to the death at level 2 with Thresh, the doran's shield will provide me with better defensive stats than the relic shield.
And you would level hook last because maxing it does not provide much damage or helpful stats. Having a lantern with twice as much shield power and a flay with twice the slow amount is much more useful in a fight than a 1 sec cd reduction on the hook.

That isn't true. Full leveing on hook (by level 8) gives you 8 seconds less cooldown, not 1. It is very useful in teamfights as it is much harder cc than flay and you won't even need to use lantern more than once if you're landing hooks. Focusing on damage over cc seems misguided when the focus should be peeling for the adc (as you know), which is harder to do when you don't have a hook up.

Doran's shield is not good for playing aggressively. Yes it gives you a lot of health but relic shield gives you almost as much and allows you to heal your adc. That is a massive premium imo because, like you said, of the sustain. I play aggressively as Thresh also and try to engage by level 2 or 3. The heal on the adc has sometimes turned fights just because it keeps them alive and they do 75% of the damage.

And why on earth would you level hook last?
