i think early on, (not lvl 30 peeps) utility even with this build should be used, just avoid the mana regen & get the exp one etc. however later level i understand the switch to Def.

this being said i prefer boots first, as you are going to get sanguine lvl2-4 generaly & if ure mid thats no worries.

the nerf has still put him back a bit & i no longer get complaints in games of "OP Vlad winz" as normally even if i do very well i don't do the old AP bomb doomyness.

back to items, warmogs is awesome IMO but like everyone who uses it (allmost) you need to get 2+ like deathcap

i was also overlooking spirit visage since patch. but have picked up one or twice after boots & have not regretted

Spirit visage isn't the best first item, but the build is passable.

Pro build! i have played with him a few times and this build works!!! win pretty much everytime cause of it!
