Introduction
For the skill sequence, I always see people maxing out Leap Strike first to reduce its cooldown. I completely disagree with this because you can get way more damage output from maxing out Empower first. One point in Leap Strike at level 1 is a must, but after that put a point into Empower, then Counter Strike, and then max out Empower and then max Counter Strike. In team fights it's always essential to be able to stun and deal damage, and you only need to leap when chasing down an enemy champion.
For items I always choose boots of speed with 2 health pots and 1 mana pot. This helps a lot to stay in a lane, plus you're not worrying about wasting money on any Doran's items because Jax's build is so expensive. After I finish Ninja Tabi, I start working on the Hextech Revolver because spell vamp and lifesteal are the most essential to this build. After I finish the first Hextech Gunblade, I build Guinsoo's Rageblade. This is also a must because it stacks so perfectly with Jax's ult, making you the ultimate tower pusher. If you're being fed enough, go for the second Hextech Gunblade and you'll probably have the game done by now. If the game drags on, go for a Quicksilver Sash if you're having trouble with CC, and if not, go for a Rabadon's Deathcap.
Jax should not be an initiator in a team fight, but he can be if you're the only option. With the item build as described above, you can survive almost any fight against multiple champions with your stun + lifesteal + spell vamp. I've gone 1v4 against the enemy and gotten a quadra kill just because I take so much health with this build. If you have a tank, let them initiate so you can leap from a bush or something and disrupt the entire enemy team.
If you can get your team to focus a lane, try going solo down another lane and try to push. They will either all come and try to stop you because you can down towers so easily, or they will only send 1-2 champions against you and the rest against the rest of your team. You can easily defeat 1-2 champions with this build, no matter who they are (only exception is Malzahar, unless you get the jump on him).

When entering into a fight whether it's 1v1 or 1v2 or 5v5, Use Empower (W) and then Leap Strike(Q) into the fray. With all your dodge runes/masteries/items you should trigger Counter Strike in less than a second, so you can stun all nearby champions while your passive + Guinsoo's Rageblade start stacking up. Keep using Empower whenever it refreshes and if anybody tries to escape use Leap Strike + Exhaust to chase them down. With your dodge mastery that lets you move faster when you dodge, this shouldn't be a big problem unless they have ghost, faster boots, or movement increase items.
