I just started playing Kog'maw with this build (with tier 1 runes) and it works great against even the tankiest tanks. The only differences I make are replacing clairvoyance with flash or something else, I find clairvoyance to be really situational once you get your ult. The other difference is not taking utility mastery, I almost never make it to the mana golem or the forest at all for any length of time because with the mp5 runes and tear/archangel's my uptime is better spent pushing or defending.
Utility mastery is quite useful for early mp/5, since your seals won't actually kick in until you gain a few levels. Additionally, Clairvoyance is highly useful as a team spell and can be used to not only prevent ganks, but aid team fights as well. If used correctly, you won't need flash because you'll have excellent positioning at all times. Besides, with Kog's enormous range, you shouldn't be that close anyways.
Try getting buffs, it makes a huge difference and shouldn't take more than a few seconds of your time.
