If you want to switch some summoner abilities, or tweak the masteries and runes a little bit you should be fine, this is just the way I prefer it. And of course as you go through get any items you might need, like a health potion or an elixir of fortitude/agility.

What keeps you alive

Alright, so guys, I know that Twitch is very squishy and this build doesn't have much armor, MR, or HP but that's why I put in so much lifesteal. Trust me, the lifesteal will keep you alive along with the armor in Wriggle's Lantern. but back to the lifesteal keeping Twitch alive concept. This helps if you line your ult up so that there are minions behind the person you are targeting that way you will lifesteal alot more health. Another thing that helps you stay alive is if you are being chased, using stealth works really well, saves you alot, works even better if you are walking into bushes as you use it.

What keeps you killing >=}

With Twitch timing is really important. If you are planning on killing someone go in bushes near them, stealth, and wait for the perfect time to attack, going back to the ult and lining them up with minions, you lifesteal more. And besides, the longer you are stealthed the longer you get an attack speed buff after going unstealthed.

Some people don't realize that using Expunge when they are almost dead is the best way to use it, this is because the more stacks of your passive you put on your target, the more damage it deals (They get one stack on them for every time you hit them with basic attack or your ult). So wait till they have around 1/4 hp and use Expunge. It will just nuke them, I've had alot of people raging at me because of it :P

Pros and cons

Pros:
-Great carry, can annihilate teams in a matter of seconds.
-Is a great crippler
-Can scout for your team
-Can escape with Ambush
-Never has to go back to spawn for health with lifesteal so high

Cons:
-Squishy, easily killed
-Can be CC'ed
-Since you are a carry, everyone will be out to get you
-Oracle's and wards can stop you from scouting, and escaping with Ambush
