I agree you should get last whisper instead of malady and bloodrazor instead of nashors tooth,but your runes are almost useless because when u use ur ult or ambush you will go well over the cap(2.5) for attack speed.You should change your runes to armor pen and it will be much more effective. Try making those changes and I think this will be a good build.

i dont like ur runes all attack speed why?! twitch is one of the highest attack speed champs he got 2 abilities that boost his attack speed and i dont like nashor tooth i prefer u get infinity faster and it will be better

i recoment you core items Berserker's Greaves Infinity Edge The Bloodthirster The Bloodthirster madred's bloodrazor last whisper. If you finish this build you can own even the best tank in some sec. also is very good to take marks with armor pen you really need it

may you replace Nashor's Tooth with madred's bloodrazor and malady with last whisper for some armor pen. Malady now is useless for twitch. It gives magic pen and twitch dont need magic pen.
