The basic rule with him is not to get caught in-depth of battle. You must play with head, and if you do so, you will probably never use Heal. Of course, if you lack some survability skills, go forth and get it, it's just my opinion.
As you can see I've concentrated way more on the total damage output, meaning that I would be able to overstand most tanks as my damage is almost 50-50 and Tanks rarely stack both Armor and MR. On the other hand you concentrated more on AD. This is like 75:25 ratio. You go more in Crits, AS and AD. AP goes second, this is somehow a huge mistake. Your basic attacks will 3/4 of your outcome damage meaning you won't be that great against CC/Tanks stacking Armor, but sure Healers may get a really tough time with you.
The main problem is the Phantom Dancer, it should be the last item, always, at least for Fate. Nashor and Guinsoo along with Sheen and Hextech are always first to go. If you want to play hybrid, you just can't go more AD/Crit/AS once, because they will know how to beat you faster. Also, when it comes to Masteries, Fate is not a 100 % Offensive Champion like Yi, Trynda, Vlad or Fiddle, he is more like a Finisher/1-Hit Supporter. Nevertheless, I'll give it a shot, playing Fate in different ways is just too mach fun:)
