Basically, you're never going to use it, because you shouldn't be buying runes you aren't planning on using at level 30.
If you put in two runes of the same tier, it gives you a different rune of the same tier. So if you put in two greater runes, it gives you a greater rune, while if you put in two lesser runes, it gives you a lesser rune.
If you put in five runes of the same tier, it gives you a different rune of the next highest tier (up to greater runes). So if you put in five lesser runes, it gives you a single normal rune, and if you put in five normal runes, it gives you a single greater rune.
The actual rune it gives at the end appears to be random, so only the tier is determined by what runes you put in.
Now, it's useful for recycling old runes if you did it wrong and bought them before level 20, but other than that it's pretty much useless because you have almost no control over the end result and it always takes more runes in than it gives out.
