It COULD bug to kill you without proccing the shield, but that shouldn't be the case.
It's either that:
A) The shield will activate and from then on takes the magical dmg until it's depleted and let the remaining dmg be subtracted from your HP.
or
B) The remaining dmg (after the shield is depleted) simply vanishes into thin air, if all of it came from one dmg source, Karthus Ulti so to say.
But dunno if B) could happen, I kinda rememer that there were similar cases before.

The shield will activate. So in Der Scuple's example case B will apply.

Your champion currently has 500 HP and a Hexdrinker. At 500HP, your champion is at 40% of his Max Health. Karthus decides to troll you and pick up an easy kill, casting his Requiem, which will hit you for exactly 600 Damage.
Now my question is, does the Hexdrinker go off at all? The way I see it two things can happen,
A) Technically you do take damage that reduces your HP below 30%, but by that point you're already dead. So the shield never pops.
B) The game calculates this ahead of time and gives you the shield, which is immediately destroyed, leaving you home free with 150HP
