For me, usually a hard leash is gotten only from bot lane.
I have both of them help attack, though the ADC leaves in time to get the first creep. If the mob I am starting at is not nearly dead by then, then the support lingers an extra attack or two to further help before rushing to lane.

After a bit of time, It turns out that if you jungle Karthus, people will recognize that you need a harder leash than usual and even mid will pitch in to help you get a head-start.

@Insane-DP if you're jungling with a mana-heavy champion like Karthus, it's quite common to start at Blue. While I do like starting at Red or Rock-things, Karthus is the one champion where it's a useless buff because his q constantly interrupts his auto-attacks.

You could consider getting the toad buff, but I never play Karthus so YMMV

I've started at Gromp, Rock-Golem-Things, Red, Blue, Wolves, and Birds. Currently, here's the Heirarchy of start points:

Blue: The best place to go because if you are leashed well, you can use a Health Potion, go straight to Wolves, pop Defile for a bit after you kill one of the wolves, use your second Health Potion, and go straight to Birds and use a Heal and another Smite to get to level 3. This lets you get Stalker's Blade on your first Recall .

Gromp: The second best place to go simply for experience reasons. You'll be able to do maybe Wolves and even Birds after this, but you'll be out an important Buff that Karthus depends upon to get started. Defile Costs a lot, and Jarthus runs out of mana easily before his first item. The buff looks pretty on paper, but to Karthus it's useless.

Wolves OR Birds OR Golems: It's kind of hard to tell which one of these is better, but starting at any of these just puts you behind. Sure, smiting rock-golem-things gives you a stun buff, but you never autoattack if you're spamming Lay Waste (like you should be).

Red: By far the worst place to start for Jarthus, simply because he doesn't get the benefit of the red buff in any way. Sure, his autoattacks burn, but if you're spamming Lay Waste like you should be, your autoattacks will never, ever hit. I wish that Red buff applied to Lay Waste's damage, but it doesn't.
