A few notes: this build relys on extremely good mana management and good q stacks. IF you can't do this, this build isn't for you.
Also I don't have full rune pages so i don't know what the best runes would be for this. My runes are flat CDR and a few random ones.

This is my version of tank nasus.
4 heart of gold > sell 3 to buy warmog's and turn the last one into randulen's. From there build whatever you want. I normally go Hextech Gunblade > sunfire/guardien angel.
Your killing power comes from exhaust and wither and good stacks on Siphoning Strike early on and constantly aquiring stacks on it.

PROS:
Good tank build
Very hard to kill with ult/guardian angel
Good early game health and lategame money

CONS:
* extremely weak to madred's > counter: skip warmog's and stop HoG production and build thornmail then continue onto warmog's
* Low killing power > counter: good stacks on Q and gunblade, and good teammates
* Mana problems > counter: none really, just use wither and spirit fire very sparingly, grab golem to help. mana is a huge problem for this build.

Notes on Madred's Bloodrazor: This is a good hardcounter to any HP build the only way i've found to stop this is to skip warmog's and grab thornmail and then grab warmog's. This will gimp your HP slightly but if your team isn't feeding they won't have the complete item until you have all HoG's so it should be an easy sell into thornmail. Another thing to consider would be grabbing an early guardian angel to do the classic resurrect>ultimate.
