Why play this build?
Teemo support is a bit of an off-meta pick. You might even get flamed in champ select for picking him (seems to happen more in draft than ranked to me though). So why play him? You'll be a god of vision, you'll swing 2v2s and teamfights with the blind on your Q, but most importantly, laying 1000 mushrooms everywhere and watching your opponents step on them is FUN!

Seriously though, the vision from your R shrooms (particularly with the reveal on enemy champs from Horizon Focus) along with their ability to let you push into the enemy jungle to lay more mushrooms and wards, plus the fact your opponents may take too many red trinkets because of your shrooms everywhere, makes Teemo Support a great pick to win vision and secure objectives for your team. Enough ability haste to let you constantly spam R effectively extends your range to be covering a huge part of the map - you can be defending a turrent, helping your allies peel, softening up opponents before a teamfight, and generally being a problem for the opposing team - all without actually needing to be there at the time!
Laning Phase
During the laning phase your goal is to poke the enemy ADC with your Q, but prioritising staying out of trouble unless your own ADC is at risk. Watch when the opposing ADC will be going in for some last hits and strike with your Q just before if possible, so the blind will make them lose out on some cs and gold income.

This can be difficult against the range of some bot laners such as Caitlyn and Senna, and if poking the ADC is too dangerous to your health, fall back to poking the enemy support instead to build up your gold income from your support item.

Always be on the lookout for an engage going either way, the blind from your Q is crucial to land on the enemy ADC (and/or Exhaust) in a 2v2 to swing it your way. Don't underestimate the damage you can do with a few autos + your passive E, but always keep in mind how squishy Teemo is and be ready to get out of trouble.

You don't really come online before level 6 so aim to get as much XP as possible by staying close to the farming without putting yourself in danger. Aim to ward the drake, perhaps even killing the scuttle crab for more vision in quiet moments. Once you do hit 6, put some of your mushrooms on the approach to the drake pit from your *opponent's* side.
Mid-game
Once laning phase is over, you can really start to shine. If you're going with the recommended max ability haste build, you'll have shrooms to place, but where to place them? This is where Teemo Support really comes into his own - you're the master of vision. Mushrooms grant a small vision radius, enough to uncover bushes, but they also act as a way for you to semi-safely enter the opposing jungle. Continually litter your shrooms at choke points and places where going from A to B will auto-path through such as corners.

Your most important task is to lay the groundwork for your team to secure objectives, particularly the drake as this is fought over more frequently than baron. When there are 1 or 2 minutes left before the drake is due to spawn, this is your opportunity to turn the drake entrance and paths from the opposing jungle with wards and mushrooms. Your mushrooms last for 5 minutes which is more than long enough to set them up well in advance of when both teams will be contesting the drake, which usually means there won't be anyone camping in bushes in the jungle when you go to set up your minefield.

Roam and help out your lanes which are struggling. A quick teemo sprinting into a fight and blinding an enemy can often make a huge difference, and the shrooms are also great for peeling when your allies are being chased.

If your team is behind in terms of towers and your base is starting to become exposed, lay mushrooms directly in front of and around the towers that will come under assault. It isn't as effective to put the mushrooms in the path of the creeps, but they will help flip the wave back towards the opponent after an enemy has farmed a lane and sent a large wave your way. You might be squishy but with your Q blind and R mushrooms, Teemo is great at defending turrets, along with his movement speed to get around the map, you can be a one-yordling defense against any towers that are being threated.
Late game
When you're into the late game and approaching full build (or the opponent is there already) you're at much higher risk of being burst down to dead from a quick gank. Spawn timers have gone up to such an extent that team fighting together is now the priority. Be more wary than the mid-game of pushing into the opposing jungle to lay mushrooms, though this is still worth doing for the vision advantage.

By the mid or late game, your opponents might be running more red trinkets than blue trinkets just because they're so annoyed by your mushrooms. This also means you've won the vision battle, so be happy, you can throw out more mushrooms quicker than they can find them with the red trinket cooldown.

If and when you have inhibitors exposed or destroyed, it's time to start laying mushrooms defensively in your own base. Given a minute or two to prepare, you can turn your base into a literal minefield which can turn a last ditch defense into a game-winning teamfight.
