I acknowledge that this guide is old. I love the work, it's excellent. However, even if you have rez time reduction, Revive isn't a bad option on him especially now that they increased global ult cooldowns (my Soraka used to make Karthus cry). My only other thing is this- CODING. It will make everything look better. It looks good, but for example some people (like me :]) can't stand the large pictures messing up the line spacing. That's just me. Overall, you are excellent., +1.

your build, is pretty decent. The comments im about to make are purely how I feel about Karthus, not trying to bring down your build since it's decent.
1. Cooldown reduction Glyphs is a waste of rune space. Karthus only has 2 spells that would need CDR for, and they really don't need it. Your wall Gets a low enough CD and so does your Req. I'd rather use AP per level Runes or flat AP instead of CDR. The health isn't necessary either considering if your good at zoning you won't need it. You need to be able to hit hard.
2. Im a firm believer of a 21/0/9 Karthus. Its just a better mastery setup imo for him. But 9/0/21 is good too.
3. Other than that, niceo!

My last Karthus game using this exact build in mid-lane against a Vladimir resulted in a
20-2-10 win.

I think it is nice, but it'd be more awesome if you added pictures and some more colors to the guide for an overall better impression to the reader. :)
