I think that Madred's is highly situational. I know its the best DPS item for any DPS melee, but unless there are HP tanks, or insane HP champs, this wont benefit you too much.
Ive never been a big fan of BF Sword Items. They can be costly, and they too can be situational. Black Cleaver is great for Armor tanks, and Bloodthirster is great for the stats, but you lose them if you die. And If Udyr is a threat, they will get you lol.

Yeah, i know that madred's is better for high health champions, but in the current metagame tanky characters are the rule of thumb, it also has quite decent damage against most squishy champions [Imagine a 1500hp champion level 18 (squishy) using madred's you would achieve 60 (modified by MR) damage, not counting the bonus 30 AD. (1500x4 % =60).
And as i've said in the beginning of the guide, this is mostly an AD build, outside from BF sword items there aren't a whole lot of other items that significantly boost your AD, and AD synergizes extremely well with
Almost all characters have native armor, plus in a good team comp there will always be a tank who stacks armor, this is where black cleaver comes into play.
Bloodthirster provides you with great lifesteal and AD, lifesteal allows you to regenerate some health outside of turtle stance in a battle, and synergizes with turtle when you're using it. The stacks in the BT are just a bonus as far as i can see.
BUT, you are right, these can be situational and not so great when the match isn't going as nice as you'd hope. That's the reason why, in the items section, i've put a lot of options. The item build up there is a case of maximum damage potential, but you can't always go that way unfortunately. A build is always situational except for a few select items. that's why i encourage people to adapt themselves giving them options in the item section.
