It happened a few times to HotshotGG when he was playing Mordekaiser.
It was pretty funny cause he had just tower dived from their first tower to their inhibiter tower to get a kill and didn't realize he didn't have shield.
(His shield was stuck full, and his health was stuck full aswell.)

This happens with Rumble frequently too. Pisses me off.
