Well, I find the key is to find your niche. Like... I am probably below average skill at last hitting for how many wins I have. XD
If you were to watch me play, I tend to just try and outplay the other person and kill them without any help and snowball via kills.
Who needs CS when you can feed off the blood of your enemies amirite?
And obviously "studying" other champions as well as how people react to yours is key.
Is there any specific things you'd like help with though?

Thanks for responding, and so quickly! as in the question, I am not sure what I'm doing wrong, and need a hand with figuring that out. another thing I know not to do is play when really tired.
things I belive I need work on:
counter-picking - I don't know what counters what other than the basic ideas, malphite counters AD because he scales off armour, soraka ult counters (to an extent) karthus ult because of the global effect of both. So on so forth
Last hitting - I get the basic Idea, kill the creeps only when they are at a health level where you can one hit them, not to auto attack unless you want to push lanes etc. but I know I can improve on this.
Jungling - I get it. I really do, I know how to clear, buff timers, dragon, baron, smiting etc. HOWEVER ganking is a problem, I know what to do, but timing is a problem :\
and a few other things, I get teamfighting but usually teams let me down in this point, im not to sure.

I jungle as: Udyr, Lee Sin, Nocturne, Voli, Jarvan, malphite, tiny bit of warwick and a few others, it depends on the game.
Well a few of those are iffy with their ganks. Like Warwick is the easiest post 6. It's all about positioning (Unless you're Nocturne). If you're ganking top gank from tri bush if you're blue team, and river bush if purple team.

Well to make things easier for you botlane,
try not to overextend, even if you have the advantage.
Since then, it's easier to gank. Something you yourself
as a jungler would appreciate.
One way to practice last-hitting, is to make your own custom
game and practice doing so there. Then when you go into a real
game, try doing the same and try reach 100 CS by 20 min. While doing
this, you may practice zoning your enemy, by poking them, hiding
in the brush and/or getting your partner to do so too, so you get
an exp, CS and level advantage. Needless to say, if you have those,
winning becomes that much easier :].
Also, when jungling, you can focus on SECURING the kill, not taking it.
That way, you might be, much akin to a support, able to feed your teammates.
Some junglers they come in and even though they can see the guy is about to
die, such as from an ignite from you, take the kill anyway. Very annoying.

For example, Kassadin is a counter to many mages, not least Ahri, because he's
highly mobile, contrary to a mage like Malzahar or Fiddlesticks. Thus, Ahri has
an easy time when using her ult. But Kassadin can not only silence her, he can
riftwalk away instantly. He can also jump OVER her skillshots and thus has great
gap-closing abilitis. So if you're in lane against someone, take note of how well
your abilities stack up against however you're against, because you just might find
that they counter yours.

Reach that 100 cs/10 minutes in a custom with no opponent. Do the same against bots. Then try against people.
A large part of last hitting is being very familiar with your champion's basic attack animation. Some are slower than others. You must also time your attacks while under the turret. (Melee minions take two hits then AA. Ranged minions, you AA, turret hits, then AA again.) After that, you just need good timing to make up for your minions beating on them.
