I've had great results with Swain + Maokai, although most Maokais aren't very good.
I don't really know why it works, there's no obvious synergy that I can see, although it's incredible harass. At level 4, Swain stuns, ignites, and uses Q and E, then Maokai throws his sapling. 2 second kill from range.
Still, their ultimates have pretty terrible synergy, so I don't know exactly how it works.
One other great little-used combo though is Alistar/Galio. (I have weird ones, I know D:)
I don't know why that one works either, but I've never lost a game that way, you can push to the inhibitor in 5v5 in about 25 minutes.
