Mocking Shout and Phantom Dancers. He'll outdamage you with all his critical hits. He'll deny all your retaliating damage output with Undying Rage.
The only one who could hope to compete in an even team fight or 1v1 would be Master Yi and only if he's smart enough to kite.
Singed can't be killed once fed above your gold count. He'll just run around with Insanity Potion and never be caught. If he's chasing you... well... RUN.
Akali is a snowball champion. She needs to get ahead to do well. But once she is, it's quite difficult to stop her. She'll rampage all over your squishies with Shadow Dance and then stealth away with Twilight Shroud.

Karthus. There's little you can do to stop his f--king ult, and he always just stacks SO MUCH AP and spams his ult... I still wouldn't say he is op though, just terribly annoying.
Akali's snowball is flat out ridiculous, though. GP hits like a truck, and since trynd'snerfs rework his late game just flat f--king isn't as strong anymore.

I GOT 20/2/idk in a ranked match and the other team was fed too. They were fed and then I got even more fed than them. That proves the #1 is still #1.
