Gnar is a threat at all points in the lane, and a good Gnar will beat a good Cho'Gath in lane most of the time. Gnar's Boomerang is a lot of poke, and is a fairly short cooldown if he catches it. Also, Gnar's speed boost and jump makes him hard to hit with your skillshots. Even more, Gnar's transformation gives huge, sometimes unexpected power spikes. Time your trades well by not fighting when he is about to transform, and don't waste too much mana by throwing Q's for poke. Max W, use it and your passive to sustain and farm, and call for a jungle gank. But if he gets a Phage, he's hard to catch.
Riven
Cho'Gath can either shut down Riven hard early, or get wrecked by her immense damage. Wait until she uses the first charge of her Q before you silence her, because she'll end up wasting the remaining two charges. Wait until her shield passes to go for poke, b/c it's useless to poke into a shield. In mid-game teamfights, silence her when she goes in, and Feast her to protect your ADC. Don't waste skillshots or mana early against her, or she'll punish you for the rest of the game.
Jax
For most of the laning phase, Jax is very manageable. If you're last hitting and poking with W and E, you'll find that you always have more resources than he does. Don't give him breathing room, and keep pressure on him early while he's not his strongest. If he gets a slight advantage, or he gets a jungle gank, building Frozen Heart will minimize the impact of that advantage and will put you back ahead of him. As long as you minimize the amount of farm he has going into the mid-game teamfights, this should be an easy matchup.
Jarvan IV
Jarvan IV's love to stack high base damage at level one, which gives them high early trading potential. Definitely start Cloth-5 in this lane, but realize that you cannot spam W or Q when you don't start Doran's. You'll find that you have a lot less mana in lane before you get RoA, so conserve enough for a Q in case you get a jungle gank. If he is going to combo you, silence him to prevent what you can. Sustain through his poke with Carnivore, and watch out for a level 6 combo. If you just build a Chain Vest or 2 Cloth Armors though, then you'll have an easy time in lane.
Garen
Garen's early damage does hurt quite a bit, so start some armor and sustain through his poke by farming with your passive. If he opts for a brutalizer (which many Garen's do) then continue building RoA because health will be more valuable then armor. At level 6, you'll want to be careful of his ultimate, because the damage can be surprising and can tip the lane in his favor very quickly. Once laning phase is over and your core is done, you'll be more powerful and more useful then him all game. Peel him off of your AD carry and the game will run smoothly for sure.
Irelia
Don't underestimate the power that Irelia has in lane over you. Her true damage can be devastating if you let her attack you too much. If she is exerting complete dominance of you in lane, call for a jungle gank and hope you get one. Early on, she may not hurt too much, but she has an insane mid-game power spike if you let her farm. No one attribute (Health, Armor, AP) will counter her. A combination of all, some skill, and a little luck nets you a win in this lane.
Darius
Darius's passive is one of the most annoying DPS moves in the game, because it turns great escapes into saddening deaths. Do not get into huge early game trades with him, otherwise you'll find yourself bleeding out a lot more than you bargained for. Armor helps loads in this lane, and buy extra health potions early on. If you're behind and he's bullying you under tower, consider yourself lucky, because he can zone you very easily from that XP if he were to slow push the lane. Sometimes one early kill is enough to tip the scales in your favor for the rest of the game, so go ham if you have the guts to.
Kayle
Kayle can be somewhat annoying in lane due to her range and AOE splash magic damage, but overall, you should come out ahead in the lane thanks to your sustain. Prioritize Magic Resistance and she won't hurt much in lane. An Abyssal Scepter will give you the resistances that you need, and throw in some nice AP for trading back with her.
Nidalee
Nidalee has great amounts of early game damage thanks to the change to her ultimate in Season 4. With her ability to go into cougar form at any time, there is a great possibility of her doing more damage than you in every engagement. Max W against her and silence her whenever she tries to Pounce in, as she is extremely ability based. Grab boots early on to dodge her long range spears, and pick up some Magic Resist in the first few minutes of the game. You outscale Nidalee by a lot in the late game, so look for fights and avoid her long range poke.
Maokai
Maokai is just as tanky and durable as Cho'Gath is, the difference being that Cho brings more damage to the table. You will be able to out trade Maokai in most 1v1 engagements, and heal up much easier than he does thanks to Carnivore. Be wary of jungle ganks though, because his CC will make sure that a Cho, without any escapes, dies when that Mandrop lands on you.
Dr. Mundo
Dr. Mundo is an extremely farm reliant tank, who requires items to get to his indestructible state. Build RoA and Abyssal Scepter first and you'll find that every skirmish with him puts you at a higher health and him returning to base. Don't take too many cleavers to the face, otherwise he will bully you out of lane.
Pantheon
This is the hardest matchup you will ever play against as Cho'Gath. Pantheon gives you hell early on in the game, especially if he builds a Crystalline Flask over Long Sword and Pots. The mana that he gets from Flask will ensure that he can spam spears nonstop. Build lots of early armor, but if he builds a Brutalizer, get some health. Call for a jungle gank and kill him/force him out of lane. He has no good escape mechanism, so call your jungler and get the job done.
Cho'Gath
Cho'Gath v Cho'Gath is the most hilarious matchup in the game, because you'll probably never see it. In the case that you're playing a normal game and there is another Cho'Gath fanatic on the other team, the way to beat him is to dodge his Ruptures and Silence him before he can do the same to you. Make sure that you're always last hitting, and force him under tower, because it's easy to poke him when he's under tower with your kit.
Rumble
Rumble can either be a really easy matchup, or really annoying, depending on the person who is playing him. A good Rumble bullies you out of lane very quickly if you let him, so prioritize Magic Resist, get Treads for Boots, and get RoA and Abyssal first. Once you have items, it should be easy to trade back with him, and the threat of a Feast is great, so don't underestimate your ability to fight with him. Make sure to use Carnivore to its fullest potential, as it will be a lot easier to stay in lane if you can last hit.
Ryze
This matchup is very similar to Rumble. Ryze has the potential to bully you out of lane in the first two levels, so build some Magic Resist and a Health Crystal early and you should be fine. Once you have Catalyst the Protector and some MR. you'll be just as tanky, if not tankier than he is, so land some moves and trade with him. Your level 6 combo is a lot stronger than his, but don't let him turn back around on you once your combo is down. Ryze can punish you if you miss your spells, so hold your Q until your 90 % sure that you'll land it.
Nasus
Nasus has an extremely weak early game, so exploit this by taking 21 in Offensive Masteries and Taking AP Quints for Runes. He has a lot of innate sustain because of his passive, but keep poking him with your W and E, and force him to deplete his mana pool early to farm. Denying him farm should be a priority, but don't forget to farm up yourself. Make sure not to Feast too early at level 6 because his ultimate gives him some more quick health, which can surprise you if your not prepared for it. If you shut him down in lane, he is not going to be a mid/late game threat.
Fiora
Fiora has high damage in lane, so build some armor against her and you'll be alright. Silence her whenever she goes all in and she will have to back off. If you let her poke you down, this lane will be very hard. Always keep your health high enough so that her ultimate won't one-shot you. In teamfights, peel her off of your ADC and silence her when she wants to ult everyone.
Jayce
Jayce's early game damage is heavily reliant on his mana pool, and if he chooses to try and poke you down early, it's going to cost a lot of mana and render him useless for a while. If you feel that you can't handle his potential for early damage, pick up Cloth 5 instead of a Doran's Ring. Jayce's transition into the mid-game comes easily when he has a brutalizer and a tear, and pokes your whole team down with his big blasts. Dodge the blasts, and knock him up when he tries to engage. Let him try to engage, and turn the fight on his head.
Renekton
This may seem counter-intuitive, but Cho'Gath does extremely well against this lane bully. Cho has the ability to sustain through all of his bullying, and mitigate some of it by his Silence-W. You will outscale him greatly once you hit the mid-game, you'll do more damage and you'll tank more. Get through the first 8 minutes of the game and you'll be fine.
Malphite
Malphite is a really easy matchup for Cho'Gath early on, because of his extremely small early mana pool and his inability to do much damage in the early levels. With your kit, you can minimize the impact of his passive by continually poking him with your E on. When you both hit level 6, you'll still be able to 1v1 him, but be careful that his engage isn't a jungle gank, otherwise you'll be staring at a dark screen for a few seconds.
Shyvana
Shyvana isn't that tough of a matchup. Last hit through her AoE damage and you won't feel too much of the damage. Put an emphasis on your own last hitting, and try to scale better into the late game than she does. If she goes tanky, then wait until she engages and peel her off your ADC in fights. Make sure that she dies before you in a fight and you should come out on top.
Lee Sin
Lee Sin has always been notable for his high early game kill potential, and he should be cautioned because of that. Try to stay behind your minions in order to avoid his Sonic Wave, and don't stay and fight him when he uses his E to slow your Attack Speed. If you do, you'll probably lose the trade. However, you do outscale Lee by miles. Once you hit mid game, protect you carries by body blocking his Qs, and silence him when he jumps in. Kill him quick and fast so he can't dance around with his mobility.
Rengar
Rengar has great damage and should be respected for that. He snowballs mostly on kills, so losing 6 farm is probably better than dying to him. Call for a gank if he gets out of line early on. You don't want him to be a threat in the mid-game, let alone at lvl 3.
Vladimir
Vladimir's Spell vamp will drive you crazy once he gets his Hextech Revolver, but before that he is very squishy and vulnerable to your damage. Keep in mind that your Silence puts a halt to him for a few seconds, and chain that with your knockup for effective CC and damage. He gets really tough to kill once he gets spellvamp, and that fact that he has range on you is tough enough. Call for a jungle gank if you can't get an advantage on him.
Wukong
Wukong is very similar to Maokai, except with AD and a little more damage. Build some armor against him and he won't even tickle you. Silence him when he dives in on you, or take a guess and silence somewhere when he goes invisible to prevent him from dashing at you. A neat trick to make him useless is to let him go invisible, let him dive in, and silence him before he can activate his ultimate in a fight. He'd be standing in the middle of your team and be unable to do anything, making for an easy kill.

I have been playing League of Legends since the Preseason before Season Two, back when Heimerdinger could press W and kill three people at once and when Blitzcrank was banned in every ranked game no matter the elo.
My champion of choice is, and has always been, Cho'Gath, because his kit allows for the most diversity in build paths, he has crazy tons of CC (Crowd Control), and he has the NOM NOM NOM! He has been undervalued in the meta for a long time, because of the presence of High Mobility champions (such as Lee Sin or Lucian), but he does crazy amounts of Magic Damage as well as True Damage with Feast, making him incredibly powerful. Hopefully with some new additions and changes to the meta in Season 5, we will see the Cho'Gath revival we've all been waiting for!
