The Screw thing on the top right, hover over it and click edit profile

Then there is the summoner section, fill your information in and press the highlighted "here" to get to the automated verifying process. Then follow the rules.

pretty sure it's a sprocket. Cogs are the notches on the wheel.

I use cog as a shorthand for cogwheel. Cogwheels and sprockets are basically the same thing anyway.

Also I used cog because I was too stupid to think of the word gear.
