So what I've found is that while I struggled initially without tear, nowadays I have pretty much exactly just enough mana for every fight without tear.
I think tear is just okay, if excessive, but even if purchased, there's no need to upgrade it to Archangel until the very end, since Orianna Support doesn't make as much use out of the raw AP that much compared to just having utility items.

Anyways, it's was an outdated comment and here I leave my latest thoughts according to my normal unranked games.
I been playing her a lot recently.
The innate AP stats Orianna gets from Sorcery or Inspiration tree incentives me to poke and oh boy, she deals a lot of damage!
To poke or not to poke?? Well.... Having 17 base ARMOR lvl 1 does Hurts! A LOT! And not only Orianna deals more damage this preseason, the enemy supp and enemy ADC do so too and since the ball will likely remain on the carry... trading can be risky.
The transcendence Rune is a MUST on Orianna since we got no more Mana regen runes. I sometiemes find myself with full mana and transcendence ready to go and so I can afford to poke for some significant damage.
Speaking of damage:
I been using spellthieves and having less troubles with mana than the yellow gold coin item, which was surprising for me, I need to further test these starting items.
Aery: is busted and the nerfs were inevitable, damage is easy to proc without even wasting mana, just autos.
Comet: havent tried, will likely underperform even after Aerie nerfs since I won't be using my abilities offensively.
Kleptomancy: awesome vs mellee supports, easy gold, offers good sustain although it is unpredicable.
Resolve tree, the Guardian keystone: I havent tried this yet and the reason is simple.... I cant afford to lose transcendence from sorcery tree and also the 5%CDR of Inspiration Tree! Maybe resolve tree isnt meant to be for me. The armor rune is sweet but not as good as the other options.
The Ultimate Hat is nice, I tried on champs like Lulu and Soaraka, but Ori doesnt benefit as much as she does from it as she does from Transcendence!
Celerity gives 4% move speed but will be likely nerfed to 3% which is the same as 2 old move speed runes.
The biscuits are really nice and though I may sometimes dont even need to use them at all so I "waste" them just to get the 40 manapool increase
Free boots are also really good, I keep taking boots or the biscuits depending on whether I feel like I would need more sustain or that the enemy team relies much on skillshots. I never take both if Inspiration isnt my Primary Tree since 5% CDR is way more important for me.
I feel like Orianna could use a "whenever she picks ball by walking into it get some mana back mechanic" to help her have better sustain while. It'd have to be less mana han the cost of a Q of course. I use the ball a lot to check for bushes and just thought of this would be a good idea.
Im loving athenes unholy grail item, it feels nice on her, the sustain is good, specially vs another mage support because of the MR stat too.

I tried the Resolve tree for a while but was consistently underwhelmed by how much healing was given by Font of Life. Even when playing in team compositions with friends specifically designed to maximize Font of Life, it was extremely meager. Its only real use came from allowing others to proc Ardent Censer, but because both Command: Protect and Dissonance now apply Censer, it was too redundant.
From there, it was Sorc + Inspiration. I settled on Sorc both because Aery is extremely effective on Ori Sup and the Inspiration keystone is underwhelming.
From there, it became a matter of deciding which mana regeneration option to go with: Manaflow or Biscuits. It was a tough choice because both have their pros and cons.
1. Manaflow provides more mana, especially in the mid-late game, but prevents getting Ultimate Hat.
2. Biscuits provide more health sustain, especially in the laning phase, but if you want 45% CDR (you do) then you can't get magical boots anymore, which is effectively 350 gold.
350 Gold + More Mana vs. More Ult Uptime
In the end I chose the 350 gold, because I found I wasn't using my ultimate off cooldown most of the time anyway. Having more mana felt more useful for keeping impact later into the game too since I found the Biscuits insufficient later on.

P.S. if you sell biscuits, you still get the 40 permanent mana.
I keep finding Coin an unreliable source of mana regen. I really do feel the difference, even when rotating I dont have minions dying near and the mana regens slowly.
From my perspective: Coin: more gold, less mana; spellthieves: arguably less gold, more mana sustain.
I didnt take transcendence much as I think it only makes mereach the CDR cap a little sooner. I took scorch most of the times.
Gathering storm feels like a late game carry option, but it also makes the shield a lot stronger in the lategame.
Scorch: its damage is significant on the laning phasa and it really adds upp with Aerie and Ori's passive.
Waterwalking: strangely I took this vs junglers like Lee, Heca, Twitch, Evelynn so I could deeply ward their jungle more safely, wont recommend overall.
UltHat: agreed with everything, its an amazing rune but it feels like a trap. I would consider it when I know for a fact I really want to build a Tear be4 the game even starts.
Tear of the goddess: when I decide to go greedy and build this item I manage to spam my abilities like a madman, it's a crazy amount of value. Seems nice in theory with transcendence since I'll be delaying my CDR aquisition anyway. I can even AFK spam W to push the waves really quick, but it also makes manaflow band feel useless lategame.
Will look furher to play speelthieves, faerie charm, rush tear and SS and see how it goes, Im usually so safe I manage to stack it and the greedy route pays off.
Celerity: What can I say, I like this rune a bit too much, more difficult to chase and catch warding and more useful the more skillshots the enemy has.

I think the issue with Tear is that if you're going for it, you shouldn't get Manaflow at all. Tear alone is just such an absurd amount of mana.
One of the reasons I like Transcendence is actually because it helps Ori Support scale much harder into the late game. It's pretty common to overcap on CDR with Ori Sup as you start getting into late game, and with Transcendence, I've actually been able to reach 400 AP just with support items. Far too late in the game to deal any damage, but it made my shields and waveclear very significant.
I find the tear very good to spam abilities or in difficult lanes to fight back with some aggrsion when the shield isnt cutting it. I'll make the tear a somewhat rare item for me and run manabond instead since it seems much more practical to run a rune than invest my first ever gold in a tear which offers no CDR on its own and ni vision and even lategame she does decent without it.
Transcendence has been nice and will use it more, I just have to get used o not have celerity or simply buy mobys more often instead of CDR boots.
The games I hadn't manabond and had the UltHat offered me more Ult chances, (duh), but the ults, though frequent, started to be a bit more lazy and careless since I had it again in a few seconds. I started to using it more offensively rather than defensively.
I think it's all about management, CDR and ma management, and she benefits more from the mana than the ult and she has a relatively short cooldown on its already as you had said it.
It's been fun experimenting with the hat, but I really have to settle down for manabond.
New runes will be introduced in the long run and AP itemisation will be updated in patch 8.03 more or less, we'll see what the future brings us.
And true, the waveclear is nice, I hadnt this luxury before pre-season. So far pre-season has been really nice for OriannaSupp.
