I also die a lot on Sejuani. My average score is 5/7/9 (Very close to 2/8/12). Which is a lot of deaths, but it pays of in kills and assists. I just try to not die early (because that can ruin a game.) On Sejuani I mostly try to avoid early skirmishes, unless I know for sure I can win them. I would never skirmish vs. an Udyr or a Hecarim. Ergo you shouldn't die in skirmishes unless you made a misplay or didn't keep track of the enemy champs. So I mostly ward the enemy jungle, and then play around the opposite side of where they are. I mostly die in teamfights.
I realise though that in order for this to work you need to have at least a great support (and jungler) who know where to ward at what stage in the game and you won't always have that. You also need your team to not get caught. Because in the end Sejuani is a team-oriented jungler, she can't carry a game, if someone else makes a bad play chances are you die too.
In teamfights it's a different story (It's also the source of most of my deaths). Ultimately deaths are not that relevant if your team wins the fights after you die. 2/8/12 is not a bad score if it means you won the fights more than 4/8 times. However I tend to play on the riskier side of things, q flash ulting in to engage. It's the ARAM mentality that I still need to work on. The key to not die on Sej is knowing where the enemies are and knowing when you can fight.
In the end, if the net gain from a play is positive, your score doesn't matter. Playing around vision is key to making more good plays. Play with your team and you should be winning most games.
