Try second item Rilay's for the slow (so horizon works) and third item horizon focus 10% more damage on ult and reveals enemies for 6 secs, zhonias is trash low ap item, just fight smarter, first strike scales super hard even if at full build Harvest does a litle more damage, you get the damage sooner with the extra gold, also ultimate hunter is the best secondary rune for more ults and more gold, you also have 3k+ health late game so you are quite tanky

jaravan IV is good as well with kartus cuz jaravan IV R

I mean, this guide is from the man that makes challengers looks like clowns himself. There's literally no better Karthus to ask for advice than him and yet he gave it all here for free. Insane.
