I just play as Jerry (the mouse from Tom and Jerry) and always roll with exhaust and flash. I make sure they never get on my booty. It's really a cat and mouse game. Kinda like being an assassin, but instead of finding opportunities/moments to execute your enemies you dip in and out of situations to help your teammates with phat heals, silences and damage reductions.
I always say, whatever works best for you, roll with that. Forcing someone to play a certain playstyle doesn't work in their favour. It's just what I found myself working the best for me.

How was Mordekaiser's room like, was it messy? Filled with metal band posters and electric guitars? Jokes aside XD Good job on escaping the Death Realm! He should've just went with Morellonomicon.. or Thornmail if your team was heavy on physical dmg.
All you gotta know is that the GW items apply 40% healing reduction. Both Chemtech Putrifier and Morellonomicon applies GW by dealing magic to the enemy champions, whilst Thornmail applies GW when champions auto attack the player with Thornmail.
All three of them can also increase the effect to 60%. For Chemtech Putrifier it's healing or shielding allies. For Morellonomicon they gotta be under 50% of their max hp. For Thornmail your enemies got to be immobilized.
There's not much you can do against anti-heal, it's a counter build item. Things you can do however is dodging abilities so that GW doesn't apply on you. That way atleast you still get full benefit out of the heals from landing Q's and using your R.
EDIT: As of patch 11.18 when using R, Grievous Wounds gets removed from all targets before applying the ultimate heal.

Answer to your question: land Q's.
XD I know it sounds troll but it really comes down to that. The more Q's you land the less often you have to reset. The less you reset the more pressure you apply to enemy towers and you gain more xp by staying in lane.
Something that may help in landing your Q's, keep in mind the further away you throw your Q the longer it travels and thus the easier it is to dodge for your opponents.
So if you're playing against champions with much longer range than you, you probably want to completely stay out of their range cos you'll end up getting hit before even attempting to do something.
If you're against champions with long range/about the same range as you. Then try to go for them max range Q's and hope you hit them.
You can probably see where this is going.. The shorter the range becomes of your enemies (or if they're completely melee) the more aggressive you can play and thus go for closer range Q's making them travel faster and therefore harder to dodge for your opponents.
Another thing that may help alot is switching out certain items for Heal and Shield power items. The more you have of it the easier it will be to sustain yourself and teammates, it still comes down to landing your Q, but you'll have to land alot less of them. At the same time, keep in mind, the more you build of these the harder Grievous Wounds counters you.
Some examples, you can replace Banshee's by Mikael's Blessing to make it a bit easier for you. Or if they have no AP dmg at all you can switch it for Redemption. If you're bad with actives or you forget about using Redemption's active and you have champions that auto attack frequently enough you can grab Ardent Censer. Or if they have like no AD champs at all you don't need Zhonya's and grab a combination of Ardent Censer/Redemption/Mikael's Blessing. Heck you can even go Moonstone Renewer instead of Imperial Mandate.
