Normally, if you go Axiom Arc, you build bruiser items afterward such as Death's Dance, Endless Hunger, Maw of Malmortius, or Sundered Sky. That means you should still be relatively tanky if you're not too far behind. In fact, my build only adds a small Eclipse shield, so it's not really build related.
To be honest, it's hard for me to really answer your question since I don't know how you approach teamfighting or what mistakes you make. However, the most common mistake I see from newer Riven players is going in first against a fully grouped team without any follow up.
You're a bruiser, not a tank, so if you go in first against a grouped team and everyone focuses you, you will die pretty fast. It's important that you only go in if you can reach their backline and if you have enough follow up and damage to actually kill them, unless their ADC wanders off alone, for example.
That's why it's important to play around your Flash timers because without Flash, it's very hard for you to reach their backline unless you have something like a Zac engage.
Again, I can't know for sure since I've never seen you teamfight, but my best advice is to take fights more slowly. Don't go in head first. Wait for them to use important spells, especially CC, and once their backline has used their spells and oversteps, then you can clean them up.
