Hmm, where and under what circumstances did this occur?
Did the thread you were replying to get locked? If a thread is locked, you can no longer reply to it. You might have clicked reply, started typing your message, then it got locked before you submitted it and this resulted in a server error.
Alternatively if your session timed out while you were still typing your message, you might have had an issue when you submitted it, though it should have sent you to the log in screen.
Those are the only situations I can think of.
