It's his most spammed skill and getting the CDR from leveling it up plus the damage boost is essential to your damage. You don't buy any damage items so that is your one attack steroid. Without it, you won't do any damage. If you play Mundo the way I do you will find yourself always having 
Blunt Force Trauma up when fighting. Also my 21-9-0 build has great impact and synergy with Blunt Force Trauma.

Is Blunt Force Trauma worth leveling up so fast? Dr. Mundo has very slow attack speed so he can't use it very effectively withought items.
