If it says 'UNIQUE' it doesn't stack if you buy two of the same item. Also, CDR is capped at 40% including runes and masteries.

Items with unique aura will stack if you have one and a team mate has one. Other than that, it will not stack at all.
EDIT: For clarification, any 2 items with "UNIQUE" on them will stack if they're not the same item. For example,
Nashor's Tooth's CDR and Ionian Boots of Lucidity's CDR will stack. However, two Nashor's Tooths will not.

What everyone said, except Tenacity. If you have 2 Tenacity items and they both give 35 Tenacity, only one will be applied, leaving you at 35 Tenacity, no matter if you have 2 different tenacity items.
