Pretty solid. A few things:

I would get at least a vamp before I finish my tforce so I have a bit more sustain.

Masteries, why the random point in improved heal?

And CDR boots are decent but aren't berzerks usually better, even on Ez?

It's 'cause I noob and don't know how to deal with Phreak's harass I tend to AFK and blame my team if that happens. As for tons of damage, there's a trinity force right? That implies tons of damage in Phreak language :p

Actually, there was a distinct lack of explaination about what i should do against Phreak constantly harassing me about how I deal "tons of damage".
