Just look at your and the enemy's champions.
If they got a fizz you should always push (fizz snowballs outta control like a mad man)
If they got something like Tf you should pick teleport, also counterganks are really damn strong so if you can you should follow.
If you let enemy mid roam and he gets him or his team fed the game is kinda over, you should try to follow unless you are really weak at ganking (morde) or you know your laner will be fine.

2. Whether you are strong enough at that stage to be able to actually make a difference or are you going to just end up feeding a kill
3. would it be better for your team to push a tower down or maybe to gank the opposite lane?
so as Kassadin or zed i will usually follow...unless i am really behind
as Kayle i will almost always follow post 6
as someone like malzahar or brand i farm
as ryze it depends on what stage of the game it is... the later it is the more likely i am to follow
if i can warm my side lane and in that time push the mid tower down ... i am staying in lane as a general rule
