Actually does happen. I was once duoq'ing with a silver player way back when I was platinum. The enemy adc was silver 1 and instantly was like "omfg **** riot matching me with plats, ggwp"

Yes, but since not everyone uses it yet they don't want the problem to spread. Lolnexus/loling are amongst the biggest elements of cancer in the league community if you ask me.
