Sadly it is fake. You can tel from the pictures, they are from S3 and the champ is most likely scrapped. Look at the Annie texture, it's the Annie before to texture update meaning it was made a long time ago. Also if you look at the ability info 3 of them say Velkoz and 1 says Vel'Koz. Most of reddit seemed to figure this out

Man why you gotta be a party pooper? Riot has their own testing server. Perhaps these images were from back then. Maybe this guys been in development for awhile. (Some champions were being designed forever). Also, knowing the nature of Riot's personal testing servers, the name spelling could very well be lazy typos, there were a lot of typos when Xelnath was showing updates of the xerath kit changes.
Im not convinced either way so far tbh.

Riot did mention exploring more "full beast" ideas though, so it does seem somewhat likely they'd go for this type of champ now.
But I do realize the images could be fake, however the evidence isn't quite damning enough for me to be sure either way.

For all we know they might still use old model annie for testing. They do use clone annies a lot in these tests. Perhaps whoever is in charge thinks the old model is funnier to use. it's not like they just scrap every old thing entirely.

Aww I don't wanna be a party pooper :(. But you might be right. In the season 4 event they mentions a quote something like "Oh, and keep an eye out for an invasion from the Outside.".
But you are right about the testing servers. Maybe instead of Velkoz they will release Kog'maws dad or Kassadin's daughter.

The champ's name is Vel'Koz and below you can see his supposed in-game model as well as his abilities.
